Claim

A claim is a form submitted to a patient's insurance plan that documents the covered services provided to a patient by that dentist (or practice) as well as the amount due the dentist in reimbursement for those services.

Most dental insurance plans perform claims. However, a dental savings plan does not. Instead, a dental savings plan provides access to reduced rates for in-network dentists and these dentists directly bill their patients without using insurance.
